## Lead: the second-season RSV list is longer, and the window opens tomorrow

AAP’s 2026–2027 RSV policy (online 2 September; November Pediatrics) keeps the first-season rule and expands who should get an antibody at ages 8 through 19 months. Newly named groups include preterm birth under 32 weeks regardless of support, hemodynamically significant heart disease, anatomic lung or neuromuscular risk, and Down syndrome or other chromosomal differences — on top of the earlier list (chronic lung disease on support, severe immunocompromise, certain cystic fibrosis, American Indian and Alaska Native children). The second-season dose does not depend on maternal vaccine or on a first-season antibody. Nirsevimab is currently the only second-season product.

For most of the Lower 48, 1 October 2026 is when clinics and birth hospitals treat RSV immunization as in-season.

## What to do tonight

1. If you have a child 8–19 months who might match the expanded list, call the clinic tomorrow and ask about second-season nirsevimab and whether the plan will cover it.
2. If you are delivering after 1 October, ask the nursery about a first-season dose before discharge.
3. Pull the Elmo teether, button-battery party gear, unanchored Aitjunz dresser, and LTD-SM23 / 7W15TN machine off the nightstand list.
